This was seen on Windows XP Athlon 64 notebook which was shipped with a AMD Northbridge and a ATI technologies Southbridge chipset. The chipset did not have any code and was identified by PC Wizard 2005 as an ATI technologies chipset without a model number. Workaround: (unconfirmed) use an active USB hub. not every hub will work though. (4-port USB hub from Belkin was once confirmed to be solving the problem, but with an incompatible Intel chipset)
